Overview

We are a home health company looking to hire a Home Health Nurse Case Manager to join our excellent team in Southern CA! This is a full-time position (M–F 8:00 AM – 5:00 PM Pacific Standard Time).

The ideal candidate must be:

Very organized, detail-oriented, and timely

Reliable with excellent English (written and spoken)

Comfortable using email, phone, and Zoom throughout the shift

Equipped with stable internet and a secure VPN connection

As a Home Health Nurse Case Manager, you will oversee daily coordination of patient care between clinicians, agencies, and internal staff. You will ensure evaluations, frequencies, and case conferences are processed accurately and on time while supporting therapists and maintaining strong agency relationships.

Key Responsibilities

Monitor evaluations, documentation, and case completion.

Track referrals and update statuses.

Send and confirm agency conference emails with approved frequencies.

Provide daily updates to the Coordinator and Scheduler.

Troubleshoot therapist issues related to scheduling, documentation, or agency protocols.

Coordinate vacation coverage and ensure continuity of care.

Qualifications

Must have 1–2 years of prior home health nursing experience (RN ) and background in case management, scheduling, or clinical operations.

Active nursing license (RN )

Proven ability to multitask under pressure while maintaining accuracy.

Excellent written and verbal communication.

Strong organizational and follow-through skills; highly responsive.

Proficiency with Google Workspace, and EMR systems including WellSky (Kinnser) and Axxess.
