Home Health Quality Assurance (QA) Nurse / Care Coordinator – 485 Plan of Care Specialist

Job Summary:
We are seeking a detail-oriented and experienced Home Health Care Coordinator / QA Specialist to oversee the development, review, and compliance of the CMS-485 Plan of Care form. In this role, you will bridge the gap between field clinicians, intake, and physicians to ensure all patient orders are accurate, compliant with CMS guidelines, and processed efficiently to support high-quality patient care.
Key Responsibilities:

• Review, edit, and track the CMS-485 Plan of Care and physician orders for accuracy, clinical consistency, and regulatory compliance.
• Ensure that the Plan of Care perfectly aligns with OASIS documentation and ICD-10 coding.
• Facilitate the timely sending, tracking, and retrieval of signed 485 forms from attending physicians.
• Coordinate with the Director of Patient Care Services (DPCS) to verify that visit frequencies match the approved Plan of Care.
• Collaborate with Intake, Clinical Managers, and field staff to resolve documentation discrepancies quickly.

Qualifications:

• Minimum 1–2 years of experience in Medicare-certified home health care workflows.
• Deep knowledge of the CMS-485 form, Medicare homebound criteria, and home health regulations.
• Active RN or LVN/LPN license, or formal COS-C / HCS-D certifications preferred.
• Proficiency with Home Health EMR systems (e.g., Homecare Homebase, Axxess, WellSky).
• Incredible attention to detail, strong organizational skills, and excellent communication.

| Hourly: approximately $5.00 per hour depending on experience and certifications like USRN)
