Diabetes Health Coach - Spanish Speaker

Work Schedule: Full-Time | Monday–Friday | 8:00 AM – 5:00 PM (PST)
Compensation: $5.00 per hr - 40 hours per week
Work Setup: Remote

We are looking for a dedicated and compassionate DM Power Coach to support individuals living with diabetes through personalized nutrition guidance, continuous monitoring, and remote care engagement.

In this role, you will empower patients to improve their health outcomes through sustainable dietary and lifestyle changes.

Responsibilities

• Provide individualized nutrition counseling for patients with diabetes
• Develop tailored diet plans and clear, actionable eating guidelines
• Monitor patient health data and progress using remote tools
• Adjust care plans based on patient needs, outcomes, and feedback
• Educate patients on diabetes management, including lifestyle and dietary strategies
• Maintain accurate documentation of patient interactions and progress
• Collaborate with healthcare teams to ensure coordinated care

Requirements (Must-Have)

• Registered Nurse (RN) or Registered Dietitian Nutritionist (RDN)
• Strong clinical experience
• Background in diabetes education and/or nutrition
• Experience with remote patient monitoring programs

Preferred Qualifications

• Experience in nutrition or dietetics with a focus on diabetes management
• Telehealth or remote patient monitoring experience
• Strong communication and interpersonal skills
• Ability to build trust and engage patients effectively
