Case Planner – Personal Injury PT Clinic

We are a growing Physical Therapy clinic looking for a reliable and detail-oriented Case Planner to join our team full-time. This role is ideal for someone with a medical background who is organized, comfortable with follow-ups, and enjoys working as part of a team.

Rate: $7/hour
38-40 hrs/ per week
Schedule: Full-Time

Responsibilities

• Gather and follow up on medical reports
• Send referrals to medical facilities
• Add and update patients in our CRM
• Track referrals, reports, and pending case items
• Make outbound follow-up calls
• Coordinate with clinic staff and other team members

Qualifications

• Graduate of a medical or allied health degree
• Knowledge of Physical Therapy
• Personal Injury knowledge or experience is a plus
• Strong attention to detail
• Organized, dependable, and proactive
• Good written and verbal English communication skills
• Comfortable making outbound calls
• Team player with a willingness to learn

A Plus If You Have

• Experience using a CRM system
• Previous experience working with a U.S.-based Physical Therapy or Chiropractic clinic
• Knowledge or experience with Personal Injury cases
