I. Home Health ICD-10 CODER Responsibilities
***Experience dealing with coding holds/problem charts highly desired

1. Accurately assigns ICD-10-CM codes from the care plans, and other applicable documentation in accordance with official coding guidelines.
2. Maintains a minimum of 95% overall coding quality score in diagnostic coding.
3. Maintains the productivity expectations as defined by the department standard for home care services.
4.Works closely with clinical team members for corrections/updates/clarifications to either the 485/POC, or the OASIS.

II. OASIS REVIEWER Responsibilities
1. Oasis Review and suggested corrections
2.Abstracting information from the Medicare OASIS D form and other medical records.
3.Reviewing the 485 Plan of Care for compliance.
4.Reviewing Face to Face Documentation for regulatory compliance.
5.Maintains a minimum of 95% overall coding quality score in diagnostic coding.

Qualifications:
-minimum 6 months recent OASIS review experience
-minimum 6 months recent experience in home health and/or hospice QA review
-Certification in home health coding or Oasis Review (HCS-D or BCHH-C in ICD-10 and COS-C) a plus
-RN a plus
