Community Outreach & Referral Cordinator

Countess Cares is seeking a motivated and organized Community Outreach & Referral Coordinator to help grow community partnerships and increase referrals for our supportive housing program.

The ideal candidate will research local organizations, hospitals, behavioral health providers, dialysis centers, rehabilitation facilities, shelters, senior organizations, veterans’ organizations, social service agencies, and other community resources to introduce Countess Cares and build lasting referral relationships.

Responsibilities

Research local organizations and potential referral partners.
Contact organizations by phone and email to introduce Countess Cares.
Schedule resident assessments and informational meetings.
Build and maintain relationships with referral sources.
Track outreach efforts using spreadsheets or a CRM.
Follow up with contacts to increase referrals.
Represent Countess Cares professionally and communicate our mission with compassion.
Assist with community outreach events and presentations as needed.

Qualifications

Excellent communication and customer service skills.
Comfortable making outbound phone calls and sending professional email s.
Strong organizational and research skills.
Self-motivated and able to work independently.
Experience in healthcare, social services, marketing, community outreach, or sales is a plus.
Proficient with Microsoft Office and Google Workspace.
