Code for a Website

I need to create and add to our website a section of questions that will walk a patient through whether or not they need to prepare for allergy testing.

For example, if a patient has concerns about nasal/sinus symptoms and has certain insurance, they can expect to have allergy testing done and should be off antihistamines for 5 days before their appointment.

The freelancer should build this so my staff can update the rules later on our own.
