We are seeking a full-time Home Health Care Scheduler (AI-Augmented) coordinate patient care in tandem with an automated AI scheduling agent. In this role, you will act as the vital human-in-the-loop, auditing automated visit placements, managing acute schedule changes, and ensuring compliance with care plans.
Key Responsibilities AI Care Coordination: Monitor and audit daily patient-clinician visit maps generated by the AI scheduling agent. Conflict & Route Resolution: Manually resolve complex booking overlaps, geographic routing inefficiencies, and last-minute call-outs.
Compliance & Care Matching: Ensure the AI agent accurately matches skills, certifications, and availability with specific patient care plans.
Stakeholder Communication: Serve as the primary contact for patients, families, and field staff regarding schedule modifications.
Maintain data integrity between the AI platform and our primary electronic medical records (EMR) system.
Qualifications Experience: Minimum 1–2 years of scheduling experience within a home health, hospice, or medical environment preferred . Software: Prior experience with WellSky or similar home health EMR software is highly preferred. Tech Savvy: Comfortable collaborating with AI tools, workflow automation software, and digital mapping systems .
Industry Knowledge: Basic understanding of home health regulations, ( HHA), and visit frequencies preferred.
Soft Skills: High empathy, calm under pressure, sharp critical thinking, and rapid problem-solving skills.
