We are seeking a dedicated and detail-oriented Care Coordinator to oversee and manage patient care processes with a strong primary focus on healthcare ordering workflows. The ideal candidate is highly organized, communicates effectively, and can seamlessly coordinate with patients, families, vendors, and healthcare professionals to ensure smooth, efficient care delivery and timely order fulfillment.
Key Responsibilities:
1. Oversee, process, and track all healthcare-related orders—including medical supplies, Durable Medical Equipment (DME), lab tests, and prescriptions—from initial placement through delivery.
2. Coordinate directly with vendors, pharmacies, and healthcare providers to verify order accuracy, process prior authorizations, and quickly resolve any delays or discrepancies.
3. Contact patients and families to confirm order details and delivery schedules, address order-related inquiries, and communicate updates on care services.
4. Review patient care plans to ensure all ordered equipment, tests, supplies, and services directly align with prescribed clinical guidelines and patient needs.
5. Schedule and monitor patient appointments, clinical visits, and care requirements to maintain overall care continuity.
6. Maintain complete, accurate, and up-to-date patient records, ordering logs, and inventory data within internal systems.
7. Track patient progress and order fulfillment timelines, proactively escalating supply bottlenecks, clinical concerns, or urgent issues to leadership when necessary.
8. Ensure all ordering and coordination activities adhere to healthcare regulations (including HIPAA) and company policies, while offering guidance to tea ---------- mbers on standard processes.
Qualifications:
Proven experience in care coordination, healthcare support, or a related role
Strong communication and organizational skills
Ability to multitask and manage time effectively in a fast-paced environment
Knowledge of healthcare compliance standards is a plus
Detail-oriented, compassionate, and professional
Proficient in Google Workspace, scheduling tools, and basic systems
Equipment Requirements:
Laptop or desktop computer
Stable internet connection (minimum of 100 Mbps)
Backup internet and power supply
Webcam
Headset or earphones
Work Schedule:
Monday to Friday, 8:00 AM – 5:00 PM PDT (OR 11:00 PM - 8:00 AM PHT)
May require participation in a rotational on-call schedule outside regular working hours. Additional compensation or time-off arrangements will be provided.
