Home Health Intake Coordinator/Staffer

Majek Rehabilitation, PLLC is seeking a detail-oriented and highly organized Virtual Home Health Intake Coordinator/Staffer to manage incoming patient referrals and ensure timely, accurate onboarding of new home health cases. This role is essential in maintaining strong relationships with partner agencies and ensuring a seamless transition from referral to initial evaluation.

The ideal candidate is proactive, responsive, and thrives in a fast-paced healthcare environment.

Key Responsibilities:

-Receive and review incoming referrals from home health agencies
-Verify patient information, insurance details, and referral documentation
-Coordinate and assign evaluations (PT, OT, SLP) based on location and clinician availability
-Communicate promptly with therapists regarding new case assignments
-Maintain and update intake tracking systems (Excel, CRM, or EMR)
-Follow up on pending referrals to ensure timely scheduling and start of care
-Serve as the primary liaison between agencies and clinicians during the intake proces
-Ensure all documentation is complete and compliant prior to evaluation
-Escalate urgent or high-priority cases appropriately
-Track and reduce missed or delayed start-of-care visits

Preferred Skills

-Knowledge of home health workflows (SOC, evaluations, re-certifications)
-Familiarity with Medicare guidelines and documentation requirements
-Experience coordinating with field clinicians (PT, OT, SLP)
-Strong problem-solving skills and attention to detail

Work Schedule
-Monday – Friday 9-5, once a week we meet at 8 for meetings. Additional hours available for overtime (normal hourly rate)
-Must be responsive during business hours (EST)

-Timeliness of referral processing
-Time from referral to scheduled evaluation
-Accuracy and completeness of intake documentation
-Reduction in missed or delayed start-of-care visits
-Communication responsiveness with agencies and clinicians
